We are an independent family medicine clinic committed to providing comprehensive, personalized primary care. We use the direct primary care model to provide our patients with unhurried, collaborative visits -- in-office or virtually -- when they need them.

Dr. Slowey was trained in full-scope family medicine at OHSU, and while he doesn’t deliver babies or work in the hospital anymore, he practices broadly in the outpatient setting.
He provides comprehensive primary care to everyone from infants to seniors, including physical exams, acute and urgent care, in-office procedures, mental health and ADHD care, gender affirming care, treatment for substance use, and more.

DPC is an approach to primary care that’s both innovative and has deep roots in the origins of medical care. It steps outside the costly, inefficient insurance model, and lets patients and their primary care physicians work together.
For a single, flat fee, DPC patients receive all of the primary care they need. Our About DPC page has more information, and you can always reach out directly with any questions!

No! There is no enrollment fee, and soon we’ll be set up to offer a free, zero-obligation 20-minute meet-and-greet so you can have all of your questions answered before deciding whether this feels like a good fit for you.
If you decide to join, there’s just the monthly or annual fee, which you can cancel for a pro-rated refund any time. If you decide to rejoin later, there is a $250 re-enrollment fee, and re-enrollment is at Dr. Slowey’s discretion.

Yes! Per federal legislation, as of January 1, 2026, you can use your Health Savings Account (HSA) or Flexible Savings Account (FSA) to pay for your Madrona membership. Please talk with your financial advisor about any specifics!

Yes! In Oregon, per recent state legislation, if you have a health plan regulated by the state of Oregon (other than “HMOs” that keep you exclusively within their system for care), you can request that your monthly membership fee is credited to your out-of-network deductible. Please talk with your financial advisor about any specifics!

It’s our mission to do as much as we can for the membership fee. We perform most in-office procedures at cost, meaning the only charge is for the supplies or related lab fees (like pathology testing with skin biopsies). We will always provide any cost information to you in advance, and work with you to find constructive solutions.

Yes! Getting you your care when and where you need it is really important to us. Home visits have an additional cost of $200 / hr, with a 1-hour minimum, to cover travel expenses and time away from clinic. Dr. Slowey can also be flexible in meeting you at school or at work, as availability allows, for support with urgent issues, for a flexible fee. Reach out! We want you to feel truly connected to your care.

We provide comprehensive primary care, but recommend you have health insurance for things like specialist visits, hospitalizations, surgeries, and emergency care. PPO (preferred provider organization) and POS (point of service) plans work best with DPC, as there are no restrictions on testing or referrals. HMO (health maintenance organization) plans are highly restrictive of where you can get care outside of the network, and are not ideal with DPC.
High-deductible health plans -- with an HSA -- within a PPO or POS can be a particularly good match. Some DPC patients choose instead to participate in a nonprofit medical cost sharing plan. We’re always happy to discuss with you at a meet-and-greet visit.

No problem! We don’t bill any insurance, including Medicare, but you can use it for your other health care -- medication, testing, specialist and emergency care -- without issue. Medicare requires that we have you complete a one-time form confirming that neither of us will bill them directly for our services.

Medicaid requires that we discuss your alternatives to getting care at Madrona, and that we complete a form confirming that neither of us will bill them for our services. The DPC community in Oregon is working with the state legislature in the hopes of more closely partnering with Medicaid in the future. Stay tuned!

As above, we recommend health insurance to complement our comprehensive primary care, but absolutely do not require folks to have it, and understand that the world we live in leaves lots of folks without good options. We will knock ourselves out to help you access the best possible care without insurance if you don’t have it.

We work with you to ensure you have the best access and lowest costs possible for any necessary testing. One of the many lousy parts of insurance-based primary care is that you often end up paying out-of-pocket for labs and imaging at inflated insurance-only prices.
If you have insurance, you’re absolutely welcome to use it, and we’ll help you make the most of it. If you don’t have it, or prefer not to use it, we’ll work with you to access discounted cash pricing for testing, often at lower prices than the insurance out-of-pocket costs.

The DPC model lets us know our patients better, see them more quickly, and provide more comprehensive care. This means we can often provide a broader range of primary care services than is typical, and can reduce the number of specialist referrals needed.
If a specialist referral is needed, we will connect you with trusted colleagues that will bill your insurance directly or provide up-front cash-pay cost options, whatever you prefer.

If you have health insurance, you can use it for your medications without any changes. We always recommend local pharmacies rather than the giant chains, but you get to decide. If you don’t have insurance, or would like to explore other options, many of the most commonly-used medications are available as very low-cost generics, or at significant discount via online pharmacies. We can help you explore your options.

We strive to provide substantially faster and better access to your doctor than the typical primary care practice, but cannot guarantee 24/7 availability. You can always reach out directly via text or phone, and we’ll do everything we can to address your needs promptly. We want to maximize your well-being, and we know you support us in maximizing ours, too!

Yes! We offer flat-fee visits for urgent care, sports physicals, and certain in-office procedures for $200. If you decide to join Madrona within 30 days of an urgent care visit, we’ll apply your flat fee to the membership.
